Job Description:

The Purchase Area Development District (PADD) is seeking a qualified individual for the position of Homecare Program Coordinator.  Responsible for all requirements of the Kentucky Homecare program; oversight of waiting list; client assessments; compiles reports; conducts ongoing evaluation of caseloads.

Applicant must meet one of the following qualifications: 1. Have a minimum of a bachelor’s degree in a health or human services profession from an accredited college or university. Four (4) years or more experience as a case manager. With one year of experience in health or human services, or the educational or experiential equivalent in the field of aging or physical disabilities. 2. Be a currently licensed RN who has at least two years of experience as a professional nurse in the field of aging or physical disability 3. Have a Master’s degree from an accredited college or university which serves as a substitute for the experience required above.

Skills/Keywords:
Skills required include excellent verbal and written communication, initiative and self-direction, excellent organizational skills and attention to detail, and knowledge of senior and/or disability services.
